[GH-56] Regular SubBlock Model

• Implement RegularSubBlockModel, as laid out in the prototype sketch here: https://github.com/gmggroup/omf/blob/dev/notebooks/omf_cbi.ipynb
• Move old VolumeGridElement to TensorBlockModel
• Pull out shared block model orientation properties / indexing functionality
