User editing Patient record should be able to enter a new Physician

Currently, the Healthcare Choices section only provides search/selection for Preferred Care Provider.

If a matching provider is not found, the name typed in should be used to create a new Physician record, and a phone number field should appear so that the phone number can be entered for the new record.
